WELL BABY
During the PL-1
year, the resident will spend a one-month block seeing patients
in the well baby nursery, functioning as the primary care physician
for these children. The focus will be on obtaining an appropriate
perinatal and neonatal history, performing an accurate physical
exam (looking for signs of illness or congenital malformations),
well newborn care, and parent education (providing anticipatory
guidance to the new families). The intern will be attending
neonatal lectures and teaching rounds with the more senior residents,
attending staff and Pediatric Nurse Practitioners. The PL-1
will become familiar with the initial evaluations and treatment
of the common Level 1 maladies.

Hours: 8:15am
- 4:00pm
Hints & Expectations:
8:15-12
noon well baby rounds (rounding with attending begins at 8:15am)
1 morning/month attend lactation class at 10 am
2-4 PM 2 afternoons/month at the rehab clinic
2 afternoons/month at lactation rounds
4 afternoons/month at continuity clinic
All remaining afternoons doing transitional care
with NRN
First week: Become familiar with nursery protocols (polycythemia,
hypoglycemia, R/O sepsis). Copies of protocols are to be given
to the residents.
· Become familiar with feeding issues in newborn
· Achieve competence in performing newborn exam, to become familiar
with normal verses abnormal findings.
· To learn about discharge instructions, establishment for outside
follow-up.
· To be responsible to obtain census and find out which babies require
newborn exam and which are to be discharged.
· First week lecture: Polycythemia, R/O sepsis, Hypoglycemia, DR
Resuscitation
Second Week: To being infant exam and chart work, obtaining
lab results
· To be able to assess babies for serum bilirubin.
· To follow border babies and discuss their history, findings and
medical management with attending.
· Become more comfortable regarding anticipatory guidance to families
regarding their questions and discharge planning
· Second week lectures: Management of mothers with HBSag, Management
of HIV+ mothers.
Third week: Present 30 minute presentation on topic of choice
· Further skills regarding assessment of jaundice, need for lab
work, consultations
· To spend 1 afternoon with well baby attending in NICU to see interesting
cases and to do procedures
· Third week lectures: birth injuries, IUGR
Fourth week: Prepare 20-minute presentation on topic mutually
agreed upon by attending/resident.
· Responsible to round on, and decision making for border babies,
update families by phone or in person.
· Responsible for all discharge admission exams with presentation
to attending (all orders to be cosigned by attending before it is
given to secretary). Discussion with families will be initiated
by resident regarding f/u care, questions, and referrals.
· Verbal orientation for upcoming month's PL-1
· Fourth week lectures: post term pregnancy, IDM, Perinatal issues.
Other: Do not attend morning report when post
call. Do not need to wear a white coat. Read the articles
in the black manual in the NICU resource room (they rate highly).
